Government Childcare Benefits have recently changed. The Childcare Subsidy is now available for eligible families. Under the new scheme subsidies are no longer paid to children 13 years and over. Please ensure that your centrelink profile is linked to your MyGov account, as this is the only way to receive the new CCS. You will need to complete a current assessment for Childcare Subsidy. For clarification please contact the office.

Authorisation for Excursion Form:

Our service has organised an excursion for your child to attend. All of the relevant details are provided below for your convenience. The service has conducted an Excursion Risk Assessment which is available for your review upon request. For your child to attend, you are required to read this information and complete the Authority for Excursion Form. To complete this Form, you must be listed on the child’s enrolment record as either their parent or an Authorised Nominee.

  • 23rd December (Girls 4 - 6) - Supatramp, 5/2 Romet Rd, Wodonga
  • 24th December (Girls 8-11) - Flip out, 998 Nowra St, Albury
  • 2nd January (Boys 2 - 6)  - Waves Aquatic Centre, Hedgerow Court Wodonga
  • 8th January (Boys Gr 4 - 6) - Albury Indoor Rock Climbing 15 Hudson Cres, Lavington, NSW
  • 8th July (Boys Gr 7-11) - Mt Buffalo

Reason for excursion: Excursions during vacation care are for children to develop an understanding of belonging to groups and communities and to learn to interact in relation to others with care, empathy and respect. The excursions create a fun learning environment with physical activities.

Transportation Details (to and from): The most convenient and safest route as determined by the bus driver at the time. They will be going by private charter bus. All buses are equipped with seat belts. 

Staff to child ratio: Will be a minimum of 1:15. Up to 90 children with 6 staff and 9 volunteers.

The Period the Child will be Away from the Service Premises Children will be off premises for approx. 3 - 4 hours.

Authority for my Child to Attend the Excursion:

By signing the Authorisation for Excursion Form, I agree to and understand the following:

  • My child has my permission to attend the excursion listed.
  • I am listed on the child’s Enrolment Form as a Parent or Authorised Nominee.
  • I have read all of the details provided by the service and understand that I can access the Excursion Risk Assessment at the service.
  • The contact details, including all emergency contact details, listed on my child’s Enrolment Form are up-to-date. If not, I have provided the updated contact details above.

Collecting your child from the Yeshivah Beth Rivkah VAC CARE premises

We recommend all children are to be picked up by a Parent or an Authorised Nominee directly from the Yeshivah Beth Rivkah VAC premises. An Authorised Nominee means a person who has been given permission by a parent or family member to collect the child from the education and care service.

If you would like for your child to walk home by themselves, or to a location to be picked up, in accordance with Regulation 99(4)(b), they will need to be signed out by a Yeshivah Beth Rivkah VAC staff member and/or volunteer before exiting the premises.

 I authorise Miri Lipskier, Menachem Lipskier, Dina Kahn, Moshe Kahn to act as my authorised nominees. I hereby give any of these nominees consent to sign my child out of Yeshiva Beth Rivka Vacation Care, and care for my child from the cessation of the service’s licenced operating hours until the service reopens. I additionally  consent for them to authorise medication,  medical and dental treatment, hospitalisation, ambulance transport and treatment and excursions on my behalf. I understand that this is a private arrangement.

I hereby authorise Yeshivah/Beth Rivka VAC staff and/or volunteers to photograph my child and use the photographs for any Yeshiva Beth Rivka VAC for the following purposes:

A $50 non-refundable deposit is required to complete your registration

  •  Cancellations in the lead up to camp incur $50 cancellation fee per child.
  •  Cancellations for camp must be in writing, no later than 7 days prior to the start of camp;
  •  Children who leave camp early for any reason other than a medical reason will not be refunded. Any early departures need to be booked in with the admin staff prior to the commencement of camp.
  • Children who leave camp early based on a medical reason or due to a family emergency will be refunded at the discretion of the Camp Director.

Payment will be processed after childcare deductions have been calculated. If you choose to pay by another method leave the credit card form blank, however your child's place will not be confirmed until your payment has been received.(There is a 3% surcharge for American Express payments.)

I hereby authorize Camp Gan Israel chaperons or leaders to obtain any medical care necessary for my child. I understand that in the case of an emergency of any significant illness or injury, attempt will be made to contact myself when practical. I agree to pay for any cost that may occur as a result of the injury/illness. I acknowledge my child will be participating in camp activities within and outside the camp grounds. I authorize my child to participate in these activities. I agree to pay for reckless damage done by my son/daughter in camp. I authorize CGI to photograph my child and use the photographs at their discretion.

I look forward to another amazing experience in Camp Gan Israel and will give it all I've got. I understand that a as a camper of Gan Israel (the greatest camp in the world), I will behave in an appropriate manner at all times; respecting camps' staff, schedules and property. I am aware that absolutely no raiding is permitted during came, nor is bullying of any type. I will wear a Yarmulke (Kipa) and Tzitzit (for boys) and I will be dressed in a modest manner with skirts below my knee, knee length socks and elbows covered (for girls) at all times during camp, proudly displaying that "I'm a Jew and I'm proud."

I will not bring any of the following to camp: cigarettes, laptops, alcohol, DVD players, shaving cream, knives, video Ipod, cards, fireworks, fire making devices, Ipod with non-Jewish music, water guns, non-Jewish books/magazines, chewing gum and any other dangerous items. I accept that in the event any such item is found, it will be confiscated and not returned. CGI staff has the right to search my luggage. CGI takes no responsibility for any lost or damaged items during camp, including valuables. The camp has complete authority to send me home if I do not obey the camp rules at my parents' costs. I acknowledge that I must pay for any costs that may occur as a result of causing reckless damage.
